HCBS Intake Coordinator

Location: Remote (New York City Preferred)
Employment Type: Full-Time

About the Role

We are seeking an experienced HCBS Intake Coordinator to lead the intake and enrollment process for children and families seeking Home and Community-Based Services (HCBS). This is not an entry-level position. The ideal candidate has extensive experience with New York State Children's HCBS, understands the Children's Waiver, and has established relationships with Care Management Agencies (CMAs) and other referral sources.

This individual will play a key role in growing our HCBS program by building referral relationships, navigating complex eligibility requirements, and ensuring children are connected to services quickly and efficiently.

Key Responsibilities
  • Manage all HCBS referrals from initial inquiry through enrollment.

  • Develop and maintain strong working relationships with Care Management Agencies (CMAs), Health Homes, hospitals, pediatric providers, schools, and community organizations to generate and maintain referrals.

  • Coordinate closely with Care Managers to facilitate timely enrollment into Children's HCBS.

  • Demonstrate expert knowledge of Serious Emotional Disturbance (SED) and Medically Fragile (MF) eligibility pathways.

  • Coordinate and facilitate CFASS assessments and all required eligibility documentation.

  • Assist families in obtaining Children's HCBS services, including Respite, Community Habilitation, Caregiver/Family Supports and Services, and other Children's Waiver services.

  • Ensure all Medicaid, HCBS, and Managed Care documentation is complete, accurate, and compliant.

  • Track referrals, assessments, authorizations, and enrollment timelines.

  • Maintain accurate documentation in the agency's electronic record system.

  • Serve as a knowledgeable resource for families throughout the intake and enrollment process.

  • Work collaboratively with leadership to strengthen referral relationships and grow the HCBS program.

Required Qualifications
  • Minimum 2 years of direct Children's HCBS intake experience.

  • Extensive knowledge of the New York State Children's HCBS Waiver.

  • Demonstrated experience navigating SED and Medically Fragile eligibility processes.

  • Proven success obtaining HCBS Respite, CFASS, and other Children's HCBS services for eligible children.

  • Established professional relationships with Care Management Agencies (CMAs) and other referral partners.

  • Strong understanding of Medicaid, Children's Health Homes, Managed Care Organizations, and Children's Waiver requirements.

  • Excellent communication, organization, and relationship-building skills.

  • Ability to independently manage a high-volume intake caseload.
