Job description

* This is a remote position, but will need to attend trainings/meetings in the Des Moines greater metro. *

Shift: Monday - Friday 8:00 AM - 5:00 PM (weekend hours are Saturday - Sunday, 8:00 AM - 4:30 PM)

The UPaH Patient Scheduler is responsible for coordinating and maintaining patient visit schedules for all Home Health and Hospice disciplines. This role ensures timely initiation of care, supports clinician productivity, and maintains compliance with CMS and other regulatory guidelines. The Scheduler works closely with Intake, Clinical Leadership, and hospital partners to ensure efficient patient flow and a positive care experience.

  • Coordinate and manage patient visit schedules for all Home Health and Hospice disciplines, ensuring timely initiation of care and adherence to all CMS and regulatory requirements.  
  • Maintain ongoing patient schedules throughout the duration of services, adjusting visits based on patient needs, clinician availability, geography, drive time, staffing levels, and productivity expectations.  
  • Collaborate with Intake, Clinical Leadership, and hospital liaisons to facilitate smooth transitions from referral to Start of Care and to anticipate scheduling needs based on expected patient volumes.  
  • Schedule all disciplines ordered for a patient in compliance with CMS guidelines and timely initiation of care regulations.  
  • Monitor patient hospital stays and determine the need for schedule adjustments, transfers, or discharge from services.  
  • Maintain and update Epic scheduling templates for all Home Health and Hospice clinicians.  
  • Monitor and respond promptly to multiple communication channels to ensure patient and clinician scheduling needs are met.  
  • Identify and report staffing needs or imbalances to regional leadership and coordinate with PRN staff as necessary. 

Education: 

  • Associate degree in business, health administration, related field, or equivalent experience required. 
  • Bachelor degree in business, health administration, or related field preferred. 

Experience: 

  • Two (2) years of scheduling or customer service experience required. 
  • Basic knowledge of computer applications required. 
  • Previous health care experience preferred. 
  • Experience with Microsoft Office software applications – Word, Excel, Outlook, PowerPoint and Adobe preferred. 
  • Typing ability of 50 wpm and kph preferred. 
  • Completion of all physical capacity testing; if applicable.
