As a Licensed Professional Counselor, your primary responsibilities will include:

• Collaborating with primary care providers to identify, assess, and manage the mental health needs of patients.
• Conducting thorough patient intakes, focusing on diagnostic evaluations and providing tailored recommendations for treatment goals.
• Delivering brief, solution-focused interventions to patients in need of mental health support, ensuring evidence-based methodologies are utilized.
• Monitoring patient progress and initiating follow-up conversations to assess ongoing treatment effectiveness.
• Developing and implementing relapse prevention plans for at-risk patients.
• Coordinating care with external treatment providers, ensuring continuity and coherence in patient mental health services.
• Documenting patient progress and treatment plans in compliance with Agency and Federally Qualified Health Center (FQHC) guidelines, while maintaining confidentiality.
• Educating patients on self-management techniques and skills necessary for active participation in their recovery journey.
