• Conduct comprehensive psychosocial assessments of patients entering the detox program, focusing on their emotional, social, and financial needs.
• Work collaboratively with medical teams to create tailored care plans that address the biopsychosocial aspects of addiction treatment.
• Provide individual and group counseling sessions, where you will utilize therapeutic modalities to address issues related to grief, adjustment, and coping strategies.
• Coordinate care with community organizations to ensure seamless post-discharge services are in place, thus facilitating continuity of care.
• Deliver emotional support and educational resources to patients and their families regarding addiction treatment options and recovery pathways.
• Perform crisis intervention and offer counseling in high-stress situations, ensuring the safety and well-being of clients.
• Develop and implement effective discharge plans, ensuring patients are connected to appropriate community resources to support their long-term recovery.
• Participate in team meetings and hospital committees, providing insights and expertise on social work practice to enhance patient care initiatives.
• Maintain thorough and precise documentation in compliance with organizational policies and legal requirements to support patient care and outcomes.

You will primarily work with individuals experiencing substance use disorders seeking help at the Detox Center. This patient population often faces various emotional, psychological, and social challenges, and your role as a Clinical Social Worker will be crucial in helping them navigate their recovery process. Understanding the complex nature of addiction and employing trauma-informed care approaches will be central to your success in this position.

• A Bachelor's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• Two years of experience in a healthcare setting, particularly in areas focusing on addiction and mental health, is preferred.
• Familiarity with mental health terminology, treatment modalities for addiction, and crisis intervention techniques is crucial.
• Excellent communication, interpersonal, and collaboration skills are required to work effectively within multidisciplinary teams.

• Previous experience working in addiction treatment environments, especially in detoxification settings.
