As a Licensed Mental Health Counselor, your key responsibilities will include:

• Conducting thorough mental health assessments for patients in hospice care, evaluating psychological needs through empathetic interviews and standardized assessment tools.
• Developing and implementing individualized treatment plans that align with each patient's unique emotional and psychological aspects of end-of-life experiences.
• Delivering tailored therapeutic interventions using various evidence-based modalities such as Cognitive Behavioral Therapy (CBT) and supportive psychotherapy.
• Facilitating both individual and family therapy sessions, enabling patients and their loved ones to communicate openly about grief, fear, and any unresolved issues.
• Collaborating with interdisciplinary teams, including hospice nurses, social workers, and physicians to ensure holistic care and smooth transitions in treatment.
• Providing crisis intervention services and responding promptly to mental health emergencies as they arise.
• Conducting educational sessions for families regarding coping strategies and resources available during the hospice journey.
• Maintaining accurate and confidential client documentation as per legal and ethical standards using electronic health record systems.
• Participating in regular team meetings, case reviews, and continuing education workshops to stay abreast of best practices and advancements in mental health.

Your primary clients will include patients receiving hospice care, their families, and caregivers. This population is often dealing with complex grief, anxiety, and emotional distress, which requires a sensitive and informed approach. You will be instrumental in helping them navigate mental health concerns related to chronic illness and end-of-life transitions, fostering a safe space for expression, reflection, and ultimately, acceptance.

To be considered for this role, candidates must possess:

• A current and valid license as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or an equivalent mental health licensure.
• A Bachelor’s degree in Social Work, Psychology, or a related field; a Master’s degree preferred.
• Minimum of two years of post-licensure experience in a mental health setting, especially within hospice or long-term care systems.
• A strong understanding of grief processes, trauma-informed care, and effective therapeutic modalities appropriate for end-of-life scenarios.

We highly value candidates who have:

• Experience working with terminally ill patients and their families within the hospice or palliative care context.
• Proficiency in providing telehealth services and utilizing various digital tools for remote counseling.
