This position is completely remote, designed to provide our Social Worker with the flexibility to work from home while still engaging directly with patients via telehealth services. Our collaboration tools and telecommunication technologies facilitate seamless communication with both patients and colleagues. Our multidisciplinary approach includes regular virtual meetings with healthcare professionals to ensure comprehensive care coordination.

• Conduct thorough psychosocial assessments of both patients and their families to pinpoint emotional, social, and environmental factors that may impact their health and well-being.
• Develop and implement tailored intervention plans that utilize a variety of social work techniques to address identified needs.
• Collaborate with a multidisciplinary healthcare team, participating actively in case discussions and care planning to address the holistic needs of the patient.
• Facilitate discharge planning, ensuring smooth transitions from hospital to home by communicating with healthcare providers, insurance companies, and external resources.
• Provide education and counseling services to patients and families regarding adaptation to illness, encouraging empowerment and positive coping strategies.
• Utilize crisis intervention strategies as necessary, providing both brief and long-term therapeutic support to individuals, families, and groups.
• Serve as a key educational resource to other healthcare professionals, presenting on psychosocial best practices in multidisciplinary settings.
• Participate in the on-call rotation, responding to after-hours calls to assist the facility and provide urgent support as needed.
• Engage in community outreach, contributing to educational initiatives, research, and volunteer opportunities aimed at supporting cancer survivors and their families.
• Contribute to professional development by mentoring interns and participating in training programs that advance knowledge in oncology social work.

The LCSW will primarily work with patients facing various chronic illnesses, including oncology patients, their families, and caregivers. This role is integral in recognizing and addressing the psychological impacts of these conditions, providing tailored support to enhance adaptation and overall wellness.

• Master's degree in Social Work from an accredited institution.
• Current LCSW licensure in the state of California.
• Proven experience in a clinical or healthcare setting, particularly with chronic illness populations.
• Strong assessment, counseling, and crisis intervention skills.
• Excellent communication and interpersonal skills, with a genuine passion for patient advocacy.

• Familiarity with telehealth technologies and virtual care delivery.
• Experience working within multidisciplinary teams in a healthcare environment.
• Knowledge of resources and community support services for chronic illness and cancer populations.

This position offers a competitive salary ranging from USD 70,000 to 100,000 annually, based on experience and qualifications.
