Work Environment

This position is completely remote, designed to provide our Social Worker with the flexibility to work from home while still engaging directly with patients via telehealth services. Key Responsibilities
  • Conduct thorough psychosocial assessments of both patients and their families to pinpoint emotional, social, and environmental factors that may impact their health and well-being.
  • Develop and implement tailored intervention plans that utilize a variety of social work techniques to address identified needs.
  • Collaborate with a multidisciplinary healthcare team, participating actively in case discussions and care planning to address the holistic needs of the patient.
