Job description

(Only QUALIFIED Healthcare Professionals accepted) Family-NP - Community Health Center is seeking an experienced Family Nurse Practitioner for a Part Time (20 - 28 hours) Benefited Outpatient Position on Cape Cod
The Nurse Practitioner provides comprehensive outpatient care, sharing responsibilities and collaborating with the physician in the care of patients.
The Nurse Practitioner will be responsible for providing services that include assessing, diagnosing, prescribing, treating, and patient|family education.
Diagnose and Treat acute and chronic Illness. Perform age-appropriate history and physical exams. Order and interpret diagnostic and therapeutic tests relative to patients age-specific needs
Prescribe appropriate pharmacologic and non-pharmacologic treatments to patients age specific need. Monitor the effectiveness of treatments. Provide education of acute and chronic medical conditions
Manage Chronic Medical Conditions. Facilitate appointments to specialist. Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling) and Preventive Medicine
