Job description

Telemedicine Provider (NP/PA) — Remote

Theoria Medical is a physician-led organization redefining post-acute care across the country. Through our multispecialty physician services and remote patient monitoring programs, we support skilled nursing facilities, long-term care communities, and assisted living settings nationwide, combining clinical excellence with the technology to deliver it at scale.

Role Summary

The Telemedicine Provider delivers high-quality virtual medical care to residents in skilled nursing facilities, long-term care facilities, assisted living communities, and post-acute care settings. As a dedicated remote advanced practice provider, you'll manage acute and chronic medical concerns, support admissions and discharges, facilitate care transitions, and serve as a trusted clinical resource for facility partners, all through our telemedicine platform.

You'll work closely with facility nursing staff, Clinical Liaisons, and Operations leadership to ensure timely access to care, reduce avoidable hospitalizations, and improve resident outcomes.

This is a fully remote, daytime-hours position, no nights, no weekends. As we expand support for a growing national client, you may provide coverage across multiple states, active multi-state licensure (or willingness to obtain it) is required.

Key Responsibilities

Clinical Care Delivery

  • Conduct virtual evaluations for acute and routine clinical concerns
  • Assess and manage changes in resident condition
  • Provide chronic disease management and follow-up care
  • Review diagnostic testing and laboratory results
  • Collaborate with facility nursing staff on treatment plans
  • Participate in care coordination and interdisciplinary communication

Telemedicine Coverage

  • Provide scheduled daytime telemedicine coverage
  • Coordinate your daily visit schedule each morning with your assigned Clinical Liaison for an organized, structured workflow
  • Respond to urgent facility requests within established service-level expectations
  • Maintain availability during scheduled coverage hours using approved telehealth platforms

Facility Partnership

  • Build positive working relationships with facility leadership and nursing teams
  • Support facility quality initiatives and hospital readmission reduction efforts
  • Participate in facility meetings when requested

Documentation & Compliance

  • Complete all encounter documentation within required timeframes
  • Ensure billing compliance and coding accuracy
  • Maintain compliance with federal, state, and organizational requirements
  • Adhere to all telemedicine and patient privacy regulations


Who You'll Work With

Clinical Liaison — Your day-to-day operational partner. Coordinates visit requests, facilitates resident access, and assists with admissions, discharges, and onsite communication.

Regional Director of Operations — Your escalation point for operational concerns, facility support needs, and service performance.

Facility Nursing Leadership — Your clinical collaborators on the ground. Communicates urgent resident concerns and implements your care recommendations.


Qualifications

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) license
  • Minimum 2 years of experience in post-acute care, long-term care, geriatrics, hospital medicine, or primary care
  • Active licensure in multiple states, or willingness to obtain and maintain multi-state licensure
  • Active DEA registration
  • Strong clinical communication skills and comfort working in a telemedicine environment
  • Ability to work independently and thrive in a fully remote role
