PedsCare Program Assistant

Every day, we serve approximately 1,500 patients living with advanced illness, wherever they call home, be it a private residence, a long-term care or assisted living facility, a hospital, or in one of our nine inpatient care centers.

We're here to improve quality of life, to ease pain and lift burdens, and to be the Compassionate Guide that families need when time matters most. And most importantly, as the only non-profit hospice provider in the region, we never turn anyone away due to an inability to pay.

The PedsCare Program Assistant provides general support and assistance for hospice patient needs, PedsCare program office, interdisciplinary team, and pediatric family communications.

Essential Functions

  • Maintains seamless, efficient communication flow so that:
    • Relays verbal and telephone messages accurately and timely to the appropriate persons.
    • Copies and distributes organization/team written communication to appropriate team members.
    • Distributes paperwork and documentation to appropriate managers in a timely fashion.
    • Notifies appropriate Durable Medical Equipment (DME) provider of patient equipment needs as appropriate.
    • Maintains the On-Call schedule: uploads to connect and emails weekly to distribution group.
    • Notifies team of weekly recertifications due.
    • Runs a Suncoast census each week.
    • Monitor and maintain Backline for all needs.
    • Submits the CADD report weekly.
    • Corresponds with Primary Care Physician office/ hospitals as needed.
    • Communicates with the foundation for events and needs as directed.
    • Other duties as assigned.
  • Organizes and maintains patient information for team so that:
    • Tracks team census by logging all status changes, levels of care, admissions, deaths, discharges, and transfers on a change of status form.
    • Monitors the Community PedsCare email for team referral requests and family letters.
    • Scans designated documentation into Forcura.
    • Prepares IDG Agenda/ spreadsheet with recerts and inputs into Suncoast; distributes copies to team.
    • Creates the IDT meeting in Suncoast.
    • Coordinates and prepares all handouts and documents for the IDT meeting.
    • Supports PedsCare family events.
    • Works at the direction of the Community PedsCare Director with department activities, events and team celebrations.
    • Works with the team to plan and support the execution of events/activities.
    • Order needed materials, secure locations, interface with the families and help to direct the volunteers.
    • Coordinates with the vendors and participating departments.
    • Tracks all RSVP's for patient events / activities.
    • Supports the Community PedsCare event as directed.
    • Other duties as assigned.

Qualifications

Knowledge and Critical Skills

Knowledge of:

  • Must possess basic knowledge in general office procedures, computer software, office technology and medical terminology.

Skilled at:

  • Excellent grammar, spelling, telephone communication and listening skills.
  • Ability to teach, ability to evaluate situations clearly, and communicate thoughts, ideas, and assessments accurately; and the ability to interact with supervisors, employees and volunteers working within the framework of a multidiscipline team.
  • Must be able to interpret analytical information and present findings in a logical manner.

Experience

Two (2) years of administrative experience including data entry and reporting.

Experience in a health care environment preferred.

Intermediate to advanced computer skills, word processing experience with the ability to type, file and do general clerical work.

Experience with Excel spreadsheets and reports.

License / Certification

Education

Technical/high school diploma or GED.

Other Requirements

Reliable transportation.
