Job Summary:
Oversees the day to day operations of the Diabetes Self-Management Education (DSME) program at all Froedtert Health outpatient sites ensuring National Standards (NSDSME) are met and maintained at all times. Responsible for direction and development of Froedtert Health's outpatient American Diabetes Association (ADA) recognized diabetes education program. Functions as an educator, program coordinator and evaluator of diabetes specific outcomes for ADA recognition. Assist managers in achieving highest level of clinical, operational and performance excellence as well as patient satisfaction in the delivery of DSME and training. Participates in compliance and quality performance improvement efforts and incorporates the nutrition care process. Maintains professional competency and skills required for professional practice.
EXPERIENCE DESCRIPTION:
A minimum of 3 years professional, clinical experience in diabetes care is required. Experience in program coordination is preferred.
EDUCATION DESCRIPTION:
Bachelor's degree in Food and Nutrition or Dietetics OR Bachelor's degree in related field plus certification in Food and Nutrition or Dietetics is required.
LICENSURE DESCRIPTION:
Current Registered Dietitian by the Commission on Dietetic Registration, required. Current State of Wisconsin Certified Dietitian, required. Registered by the American Association of Diabetes Educators as a Certified Diabetes Educator is required. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
