Description
POSITION RESPONSIBILITIES
1. Completes intake and enrollment paperwork for individuals enrolled in the program.
2. Participates in, writes, and implements Habilitation Plans with oversight by the Program Coordinator.
3. Ensures the staff implementation of plans and documentation of daily notes and monthlies are completed correctly. Reviews for accuracy.
4. Ensures staff understand and promote the Person-Centered Approach.
5. Responsible for staff scheduling, arranging coverage and providing direct care services in the absence of staff.
6. Completes the hiring, training evaluating, and supervision of program staff including evaluating staff performance and provides feedback to staff and, as necessary, to the Program Coordinator. Ensures completion of required in-services and training for staff.
7. Provides assistance with acquisition, retention or improvement of self-help and socialization skills.
8. Develops, coordinates and assists with programmatic activities. Locates community-based events to promote integration. Encourages program participants to be good neighbors, responsible citizens, join local clubs and support their community.
9. Purchases supplies for the program as needed.
10. Manages petty cash and gas reimbursements for Day Habilitation program.
11. Uses knowledge of NFTA system and encourages mobility training where appropriate.
12. Ensure service documentation is submitted in time for billing. Completes monthly attestation and forwards it to Program Coordinator. Completes Monthly Analysis for the programs.
13. Ensures participants satisfaction and input occurs with program activities, curriculum, and goals.
14. Receives and reviews minor, reportable, and serious reportable incident reports, and follow up with Program Coordinator for direction and processing.
15. Maintains monthly contact with family and/or IRA staff. Notifies appropriate parties of concerns as they arise.
16. Ensures compliance with all OPWDD quality assurance and fiscal requirements.
17. Conducts fire safety training as necessary.
18. Monitors and manages units of service to achieve objectives for HCBS funding source. Coordinates program and funding modifications within Day Habilitation Programs.
19. Provides follow-up services and referral to discharged individuals as needed.
20. Completes data entry into Therap and assists with maintaining the database to ensure individual files are current.
SUPERVISORY RESPONSIBILITIES
Provides supervision of assigned DSP- Day Hab staff and participants of programs.
Requirements
POSITION QUALIFICATIONS AND REQUIREMENTS
1. Education and Experience
a. High School Diploma/GED
b. 3 years of Full-Time experience in OPWDD setting; with one year of supervisory experience; or 6 months of Full-Time experience in OPWDD setting and 3 years of other supervisory experience.
2. Skills, Knowledge, and Abilities
a. Knowledge of developmental disabilities and stages of skill development.
b. Knowledge of OPWDD regulations and NFTA systems.
c. Ability to facilitate group instruction, prepare curriculums and assess skill levels.
d. Professional oral and written communication skills including the ability to effectively present information in one-on-one and group settings.
e. Basic computer skills.
f. The ability to calculate figures as well as amounts and percentages.
g. The ability to read and write case notes as well as present and understand written directions.
3. Certifications - Must be or become:
a. CPR/First Aid certified.
b. Must complete the SCIP Philosophy section of the SCIP-R curriculum
4. Verifications and Screenings
a. OPWDD clearance for convictions of criminal offenses.
b. County criminal background check.
c. Clearance through NYS Central Child Abuse and Maltreatment Register.
d. A drug screen prior to the commencement of employment.
e. Tuberculosis screening started or completed prior to the commencement of employment.
f. Approval of NYS Justice Center.
5. Be authorized as an Agency Driver by
a. Holding and maintaining a valid NYS Driver's License.
b. Maintaining an approved driving record according to Agency Policy and Procedure.
1. Use of personal vehicle for work purposes.
2. Proof of being an insured driver of personal vehicle.
3. Physical and Mental Requirements
a. Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs.
b. Lifting up to thirty (30) pounds.
c. Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines.
d. Staying awake and alert while on duty.
WORKING CONDITIONS
1. Hours will usually be normal business hours; may need to work other hours to meet the needs of the program.
2. Local travel required approximately 20% of the time.
3. Provide services to individuals in their homes as well as in community settings.
