Job description

Compensation Range: $18/hr - $21/hr commensurate with experience
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.
Overview:
Coordinate all Hepatitis-C program services to individuals who are experiencing substance use and co-occurring mental health disorders and/or enduring homelessness.
Responsibilities:
  • Facilitate along with the program director the creating of treatment and supportive services
  • Supervise and coordinate a multidisciplinary team to provide evidence-based services and treatment
  • Facilitate coordination of outreach sites and plans
  • Facilitate coordination with other community-based organization and partners
  • Ensure program integration with other agency services
  • Assist in the coordination of all data gathering, analysis, and program reporting
  • Document, with attention to detail, intakes, case files, and referrals
  • Represent program and agency in different forums.

Requirements:
  • Master's in related field or related life-experience
  • Experience with coordination of programs directed to PWUD and that have experienced homelessness and/or co-occurring disorders
  • Experience with supervision
  • Experience with Evidence Based Practices / Interventions
  • Knowledge about HIV/AIDS, Hepatitis-C, and STDs in the Puerto Rican context
  • Highly motivated to work with people who experience homelessness, substance use, and/or co-occurring disorders in an outreach setting
  • Able to work closely with others in outreach team and office setting (team player)
  • Capable of executing a plan based on facts and scientific knowledge
  • Proficient and efficient with multiple computer software programs
  • Excellent written and verbal communications skills (in both Spanish and English)
  • Meticulous at maintaining records and confidentiality protocols
  • Outreach experience
  • Up to date driver's license
