Job Description:

The program Assistant at The Salvation Army Mountberry and Meadowlands Adult Day Services is accountable to the Program Services Manager to provide excellent personal care to clients. Primary functions include providing care according to the established client care plan including all components of activities of daily living and supporting participation in recreational and therapeutic activities.

The Program Assistant is required to perform their duties in a manner consistent with The Salvation Army's core values, ensure client safety, and demonstrate customer service excellence while responsible for planning, implementation and evaluation of programs and services provided to clients.

Accountabilities:

Client Services

  • Provides for personal care in accordance with established policy, the client plan of care and within the scope of practice of a PSW or SSW
  • Contributes to and follows a clan of care to meet clients' assessed needs.
  • Is responsible for maintaining accurate and complete records of personal care delivered.
  • Contributes to positive customer service with clients and their caregivers daily.
  • Deals tactfully and courteously with all personas connected with the day programs in a positive manner.
  • Facilitates communication in complex care issues to foster positive client outcomes.
  • Establishes and sustains care in a manner that is client centered and fosters evidence-based best practice.
  • Seeks direction from the Program Services Manager where clients' needs exceed experience of scope of practice.
  • Observes, reports and records client conditions.
  • Assists in maintaining an environment appropriate to meet clients' individual needs.

Essential Duties:

Toileting

  • Provides through peri care following elimination of urine or feces.
  • Follows toileting schedule as per care plan responds promptly to requests for toileting or change of incontinence products to prevent skin breakdown.
  • Accurately and timely documentation is completed regarding all client care.

Nutrition

  • Records documentation of all meals presentation and any client/staff concerns about meals.
  • Feeds as necessary considering clients' preference and dietary restrictions.
  • Provides a safe and pleasant dining experience.
  • Assist clients as required with individually wrapped portions of food, peeling oranges, eggs in shells, cutting meat etc.
  • Ording daily and monthly meals as required, in compliance with dietary needs.

Program Activity

  • May conduct and assist with planned programs in response to individual and group needs and interests.
  • Assist with the preparation of the program's physical environment and maintenance of the activity area.
  • May be called upon to present online virtual programming.
  • Contribute to the development, coordination of annual strategic long-range programs and fundraising initiatives.
  • Complete and submit incident reports as required.
  • If designated, manage program waitlist, and respond to referrals through the Health Partner Gateway. If suitability for a client participation is in question, report to Program Services Manager.
  • May be designated to support Program Services Manager in completing intake interviews.
  • May be designated to perform medication assistance duties and complete necessary related documentation.
  • May be designated to maintain Accessible Transportation bookings for clients, may support students in completing this duty.
  • May be designated to participate in Health and Safety Committee work including meetings and inspection completion and recording.
  • Maintain knowledge of program virtual documentation and maintain the ability to submit documents to the current format.

Leadership:

  • Promote positive work environment and teamwork by following the Mountberry and Meadowlands Adut Day Service's policies and procedures.
  • Mentor and guide volunteers at the program.
  • Mentor and support placement students by providing a positive learning experience.
  • Prioritize clients' needs.

Financial Accountability:

  • Ensures appropriate utilization of all supplies and equipment.
  • Utilizes assigned equipment and supplies appropriately as per policies and procedures.
  • Record and submit any donations or payments received as per Salvation Army policy.

Employee Administration:

  • Participates in an effective orientation, and performance management process in accordance with The Salvation Army Adult Day Services standards for all employees within the day programs.
  • Identifies day program's learning needs and communicates to leadership.
  • Models positive working relationships with management and co-workers.
  • Actively participates in the annual educational program for the staff of the day programs.
  • Demonstrates relevant understanding of and compliance with legislative knowledge of Employment Standards Act, and the Human Rights Act, as it applies to the direct care staff.
  • Maintains a knowledge of Salvation Army policies and practices.
  • Perform other position related duties as required.

Critical Relationship Management:

Governance Boards and Councils: None

Internal: Management, staff, volunteers, students

External: Community, client caregivers

Managerial/ Technical Leadership Responsibility:

  • Reports directly to the Business Services Manager or designate.
  • Provide support and direction to part-time and/or casual/relief staff.
  • Provide training, mentoring and orientation to new staff/volunteers/students.

Financial and Material Management:

  • Receives payment for services, documents what was received (triplicate receipt, payment binder documentation) and is responsible to deposit payments in the safe.

Working Conditions:

  • This is a permanent full-time position, 40 hours per week.
  • Flexibility in scheduling is required to meet program requirements; schedule may include days evenings, and weekends.
  • May be required to work shifts at each program location.
  • The supervisor will set the hours of work and work schedule.
  • Ability to lift/move 10 to 19kg.
  • Frequent lifting, twisting, bending, crouching, reaching, pushing and pulling.
  • Travel is associated with this job (<10%)

Education, Experience, Qualifications, Certifications, Skills:

  • Completion of secondary school diploma is required.
  • Completion of Personal Support Worker or Social Services Diploma.
  • Current Gentle Persuasive Approach training.
  • Current First Aid & CPR Certification.
  • Current Safe Food Handling Certification.
  • Montessori for Dementia Care certificate would be an asset.
  • U-first Training would be an asset.
  • Alternative combinations of education may be considered.

Experience and Knowledge Requirements:

  • Minimum of one (1) year of prior work experience working with cognitively impaired/frail elderly populations.
  • Proficiency in all Microsoft Office suite of products
  • Previous experience conducting activities including craft, music, art, fitness, games, etc.

  • Reliable transportation is required. A valid Ontario class "G" driver's license, own vehicle/insurance, and current original copy of driver's abstract, which is satisfactory to The Salvation Army, in its sole discretion may be required.
  • Provide an original copy of a background check with vulnerable sector screening that is satisfactory to The Salvation Army, in its sole discretion, is required. The screening is secured either through the national Canadian Policy Information Center (CPIC) or through a local police detachment.
  • Screening through The Salvation Army Abuse Registry.
  • Develop an understanding and support for the mission and purpose of The Salvation Army in Cananda and its implications as related to position responsibilities.

Compensation:

The target hiring range for this position is $48,048.00 to $60,070.40 with a maximum of $72,072.00.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
