Location Detail: 85 Jefferson Street Hartford (10068)

Shift Detail: first shift 40 hours a week

Job Description:


Provides holistic, quality and cost-effective nursing care for patients/families and/or others in the pre-hospitalization population within a unit, service or other primary area of practice; focuses on discharge planning which includes identification of risk factors and education. Applies the nursing process within the context of the organization's Framework for Professional Practice. Functions as clinically competent/expert. Delegates and supervises work appropriately. Reports to Director, Care Continuum.

In this position the nurse will complete the Pre-Screening Call and complete preoperative chart reviews in Epic.ย  During this call the nurse will provide holistic nursing care for patients/families/caregivers prior to hospitalization. The nurse will be responsible for accurately obtaining, assessing and identifying potential risks and barriers to expected outcomes.ย  Patient care will be provided in partnership with a provider; including phlebotomy, EKG, and other preoperative testing as ordered.ย
