Population Health Representative
Lincoln Community Health Center, Inc., a federally qualified community health center located in Durham, NC is recruiting for a Population Health Representative. Durham is known as the Bull City, the City of Medicine, the Tastiest Town in the South, and the Startup Capital of the South. It's a two-hour drive from the beach, a three-hour drive from the Blue Ridge Mountains, and is North Carolina's fourth largest city by population. Along the East Coast, Durham is located conveniently along I-85. For those outside of driving distance, travel to and from is made easy as the City and County of Durham co-own the top-ranked Raleigh-Durham International Airport that services more than 45 nonstop destinations.
OCCUPATIONAL SUMMARY
The Population Health Representative serves in an administrative role within the Department of Population Health, collaborating with all departments to strengthen the patient-clinician relationship by supporting the planning and coordination of care across the healthcare spectrum. This role is primarily telephone-based, involving making and receiving phone calls, conducting targeted outreach to engage patients, managing the hospital follow-up line, and promoting access to care. The comprehensive care provided by the Population Health Representative aims to promote better health, longer lives, higher patient satisfaction, and less expensive care.
MINIMUM QUALIFICATIONS
  • High School diploma or equivalent.
  • A minimum of one-year experience in a healthcare setting.
  • Knowledge of medical terminology.
  • Experience working for a non-profit community health clinic and/or social service organization.
  • Bi-lingual required

EXPERIENCE
  • At least one year of Ambulatory Care experience is preferred.
  • Must have a working knowledge of HIPAA and Joint Commission Patient Care Standards.
