Job description

Crouse Community Center is seeking a dedicated and compassionate Per Diem Physical Therapist to join our rehabilitation team. This role is ideal for a PT who values flexibility while still contributing meaningfully to resident care in a supportive, well-coordinated long-term care environment.
As a Per Diem PT, you will provide high-quality physical therapy services that help residents maintain or improve functional mobility, independence, and overall well-being. You'll work collaboratively with nursing, medical staff, and interdisciplinary teams to ensure each resident receives individualized, person-centered care.
What You'll Do
  • Deliver resident-centered PT services including evaluation, treatment planning, and therapeutic interventions.
  • Participate in care planning and collaborate with nursing, medical staff, and other disciplines.
  • Document assessments and progress in accordance with regulatory and facility standards.
  • Ensure safe, effective use of therapy equipment and maintain a clean, organized therapy environment.
  • Support MDS and interdisciplinary processes through timely communication and participation in reviews.
  • Promote resident safety during all therapy sessions.
  • Uphold facility policies including infection control, PPE use, and professional conduct.

Requirements
What We're Looking For
  • Current NYS Physical Therapy license in good standing.
