Responsibilities:
  • Conduct skilled occupational therapy evaluations and develop individualized treatment plans.

  • Provide therapeutic interventions to improve patients’ independence in daily living activities.

  • Educate patients, caregivers, and families on adaptive techniques, home safety, and use of equipment.

  • Collaborate with interdisciplinary team members to ensure comprehensive care.

  • Document all care provided in compliance with agency, state, and federal guidelines.

  • Participate in care conferences and contribute to ongoing care planning.

Qualifications:
  • Current California OT license in good standing.

  • Graduate of an accredited Occupational Therapy program.

  • Minimum of 1 year of clinical experience (home health experience preferred).

  • Strong communication and documentation skills.

  • Ability to work independently and manage time effectively.

  • Valid driver’s license and reliable transportation.
