A Brief Overview Assists in maintaining and coordinating practitioner and physician practice.
What you will do
• Functions as the contact person for all patient coordination needs.
• Supports all scheduling needs for the practice visits and coordinates appropriate areas for ancillary appointments.
• Follows scheduling guidelines to ensure flexibility for emergent and/or urgent cases.
• Responsible for transition of care from physicians office to Holy Name Medical Center. Is responsible for securing all necessary records/documents to ensure patient experience has no delays in services they are seeking.
• Coordinates and schedules follow up appointments via phone or in person.
• Retrieves messages from the Call Log.
• Works with different departments in assisting with scheduling patients for diagnostics testing/procedures/surgeries. Ensures that all required documentation needed to schedule these tests/procedures are completed.
• Coordinate off site visits and maintain continuity of care from offsite to Holy Name Medical Center.
• Ensures no scheduling conflicts by making necessary adjustments when priorities change.
• Distribution of mail throughout the practice.
• Confirms that all faxed progress notes have successfully been received by checking fax confirmation in XMedius.
• Retrieves faxes and distributes throughout the practice accordingly.
Education Qualifications
• Bachelor's Degree

Experience Qualifications
• 1-3 years
Minimum of 3 years of experience in healthcare
required
Knowledge, Skills, and Abilities
• Ability to identify and effectively resolve issues at the Practice and project level preferred.

• Must be able to maintain communication with staff members, practice manager and physicians.
