Job description

The Health Coverage Coordinator may be based out of the Steamboat or Craig office and will provide in-person education to help individuals apply for and enroll in health insurance and financial assistance programs, while also conducting outreach to community members and partner organizations to advance our mission and strategic goals.
We are seeking a detail-oriented self-starter to help grow this program across the Yampa Valley.

Supervision Received: The Health Coverage Coordinator may be based out of the Steamboat or Craig office and is supervised by the Health Coverage Program Manager.
General Requirements & Qualifications:
  • High School diploma required; Bachelor's degree preferred.
  • Strong written and verbal communication skills with diverse audiences, including community members, partners, funders, and government agencies.
  • Bilingual (English/Spanish) strongly preferred.
  • Highly organized with strong attention to detail and follow-through; professional, punctual, and approachable.
  • Self-motivated, reliable, and able to work independently.
  • Familiarity with community resources, organizations, and local champions.
  • Experience creating and delivering strategic communications across written, oral, and digital formats.
  • Competent with Microsoft Office Suite, Google Drive, and social media/digital marketing platforms.
  • Passion for social justice, equity, and treating all individuals with respect and dignity.
  • Valid Colorado driver's license, reliable vehicle, and willingness to travel.
  • Able to lift up to 20 pounds and navigate stairs at site/client locations.
  • Flexible schedule, including possible nights and weekends.

Key Accountabilities:
  • Assist individuals in the health insurance enrollment process during and outside the open enrollment period.
  • Support the Health Coverage Program Manager in promoting services and programs provided by the Health Partnership through participation in community, county, and regional events where outreach opportunities exist.
  • Support the Health Coverage Program Manager in identifying, and addressing barriers to health insurance enrollment to improve outreach and partner engagement.
  • Collaborate with the Health Coverage Program Manager and peers to ensure all program deliverables are being met and program evaluations are conducted.

Essential Duties:
Assist individuals in the health insurance enrollment process during and outside the open enrollment period.
  • Coordinate and provide appointments to assist individuals in the enrollment process at office and community locations during and outside of health insurance Open Enrollment.
  • Share unique skills and expertise with THP team to improve cross THP support to individuals.
  • Develop deep knowledge of government eligibility program requirements and health insurance and health service delivery is highly desirable.
  • Provide objective application assistance and explain and summarize detailed hard to understand health insurance plans.
  • Complete necessary and on-going training to be proficient at communicating access to healthcare opportunities, engagement and outreach.

Support the Health Coverage Program Manager in promoting services and programs provided by the Health Partnership through participation in community, county, and regional events where outreach opportunities exist.
  • Professionally connect with community members and partners to engage in efforts to improve overall health and wellbeing.
  • Coordinate and participate in outreach events in Routt, Moffat and Rio Blanco County and/or the northwest region to increase awareness about The Health Partnership and specifically during the health insurance Open Enrollment Period.
  • Support the Partnership's efforts to build and maintain relationships with partners, which includes attending regular meetings and serving as an ambassador/liaison between the Partnership and partners.

  • Work with low-income families, including Spanish-speaking immigrants and identify cultural, language, and learning differences to meet client needs.
  • Identify best practices and locally tailored solutions to provide health insurance enrollment information and appointments to connect county residents with health-related services, especially those experience health inequities and disparities.
  • Identify and address barriers to health insurance enrollment to inform outreach and partner engagement strategies.

Collaborate with the Health Coverage Program Manager and peers to ensure all program deliverables are met and program evaluations are conducted.
  • Identify best practices and locally tailored solutions to provide program specific outreach and engagement materials and events to support the Partnership's primary grants.
  • Document all outreach efforts and activities using appropriate methods.
  • Complete necessary and continued training to demonstrate proficiency about The Health Partnership programs including health insurance enrollment, financial assistance programs, substance use disorder and care coordination.
  • Complete necessary and continued training to support enrollment in health insurance and financial benefit programs including Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Medicaid Long-Term Care, Medicaid Buy-in for Working Adults with Disabilities, Medicaid Eligibility Appeals.

Compensation:
This is a salaried, non-exempt position eligible for overtime pay for hours worked over 40 in a workweek, and in accordance with applicable laws.
$27.40-$29.33 per hour, depending on experience.
This position is not eligible for THP health care benefits or THP's general PTO accrual program. The position will accrue paid sick leave in accordance with Colorado law, currently at a rate of one hour of paid sick leave for every 30 hours worked, up to 48 hours per year.
