We are seeking a dedicated and compassionate LVN Wound Care Nurse to join our healthcare team, serve the PHOENIX/Scottsdale Area . Our mission is to deliver advanced wound therapies directly to patients in their homes, helping them heal faster and improve their overall quality of life.
As we continue to grow, we’re seeking licensed LPNs who are passionate about patient-centered care in Phoenix and the surrounding areas.
The ideal candidate will possess specialized knowledge in wound management and will be responsible for helping assessing, planning, implementing, and evaluating care for patients with complex wounds. 
Duties

  • Conduct thorough assessments of patients' wounds, including size, depth, and condition using apps to measure. 
  • Develop individualized wound care plans based on assessment findings and best practices.
  • Administer appropriate wound care treatments, including dressings and topical medications.
  • Educate patients and their families about wound care techniques and prevention strategies.
  • Monitor patients' progress and adjust care plans as necessary to promote healing.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Document all  in electronic health records (Tebra). 
  • Prior use with Telemedicine platform helpful. 
  • Ease use with technology
Qualifications

  • Current LPN licensed in AZ.
  • Experience in wound care management is preferred; certification in wound care is a plus.
  • Strong communication skills to effectively educate patients and collaborate with healthcare teams.
  • Commitment to ongoing professional development and staying current with best practices in wound management.
  • Assess and meticulously document patients' wounds, capturing characteristics, measurements, and progress
  • Develop and implement individualized wound care plans based on patient needs
  • Administer appropriate wound care treatments, encompassing dressings,, and topical medications

  • Prior experience with Utramist Sanuwave and ABI utrasounds helpful
  • Reliable vehicle, valid driver’s license, and ability to travel locally 
  • Ability to pass background check and drug screening
