Job description

Certified Nursing Assistant 

 

Schedule: Monday-Friday 8:30am-5:00pm, with some on call requirements.

Service Area: Louisville and surrounding communities

Certified Nursing Assistant Position Summary: The Certified Nursing Assistant (CNA) is responsible for delivery of personal care and minimal homemaker services (i.e. services promoting a safe and clean patient environment) to hospice patients in the home, assisted living facility, or long-term care facility.  Services are provided based on Hosparus Health policies and according to the CNA plan of care (POC). The CNA also participates in the development of policies and procedures that guide the provision of hospice care.

 

Certified Nursing Assistant Position Qualifications: 

  • High school diploma preferred.
  • CNA certification required in the state hired to work, or an active LPN or RN license.
  • Valid Driver’s License.
  • Auto Insurance coverage.
  • One year home health aide or nursing assistant experience preferred.
  • Use of technology-based tools, including electronic medical record, strongly preferred.
