Job description

POSITION SUMMARY/RESPONSIBILITIES
In addition to functioning as a Clinical Coordinator, the team lead provides direction to the multidisciplinary clinical teams in the Mama Bexar Program. Works with management to clearly define objectives and priorities, evaluate program goals and metrics, and provide guidance in identifying and implementing process improvement initiates. Coordinates, and documents care coordination services for patients based on established criteria and procedures.
EDUCATION/EXPERIENCE
Must be a Registered Nurse. BSN is required and Master's degree is preferred. All candidates must possess at least four (4) recent years of clinical experience, which may include service coordination, case management, quality management, or managed care experience. Experience working with OB patients preferred. Experience working with EPIC EMR preferred.
LICENSURE/CERTIFICATION
Current licensure in the State of Texas as an RN is required.
