Job description

The shifts available for cases in Queens, Nassau & Suffolk Counties range from 4 - 16 hours per week (usually late afternoon to early evening during the week and a few hours on Saturday or Sunday.
You must have a valid NY State or out of state driver's license and a HS diploma/GED to be considered for this position. Most of the cases require that you have your own vehicle to use while working the case.
The pay rate for this position is $19.00-$20.42 per hour (or higher, depending on years of experience working in NYS OPWDD programs)
Some responsibilities include:
  • Participation in the training and supervision if the individual while incorporating the use of community resources..
  • Maintain on-going communication with the Community Habilitation Coordinator and attend scheduled meetings for discussions when necessary.
