Key Responsibilities
โ€ข Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
โ€ข Observe, document, and promptly report changes in patient condition to the supervising nurse.
โ€ข Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
โ€ข Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
โ€ข Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).
Qualifications
โ€ข Active Certified Nursing Assistant (CNA) certification
โ€ข Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
Field Role Requirements

โ€ข Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
