Job Summary:
The Referral Coordinator works closely with outside agencies and coordinates appointments for testing or necessary care for patients per the request of the provider that must be made outside of the Jordan Valley clinic. The Referral Coordinator may also perform administrative and certain clinical duties under the direction of a physician. Administrative duties may include scheduling appointments, maintaining medical records, billing, and coding information for insurance purposes. Clinical duties may include taking and recording vital signs and medical histories, preparing patients for examination, drawing blood, and administering medications as directed by physician.
Key Responsibilities:
JVH works within the context of a patient centered medical home and from a team approach. The team includes the primary care provider and other JVH staff, as well as the patient and the patient's family. Outside specialists and other community partners may also play a role in the team-based care. The goal of the team is to promote timely access to care, comprehension and continuity of care, as well as the enhancement of patient health. As such, your position will require you to collaborate within the team and perform duties in-line with this team-based healthcare delivery model.
  • Contact patients within 72 hours of discharge from either emergency room visit or hospital stay.
  • Perform medication reconciliation with patients
  • Advise and schedule appropriate follow-up appointments.
  • Follow-up on referrals to ensure that appointment was kept and results have been communicated back to patient and clinic.
  • Request appropriate medical records
  • Assist with scheduling referrals to specialties
  • Work reports to schedule visits for appropriate health maintenance and follow-up visits.
  • Work reports to identify non-compliant patients and reach out to those patients.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible and courteous environment.
  • Promotes effective working relations and work effectively as part of a team to facilitate the Clinic's ability to meet its goals and objectives. Utilize nursing knowledge and skills in the safe implantation of basic preventative, therapeutic, and nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Administers medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
  • Maintain and coordinate communication among patient, staff, and physician as evidenced within medical record, client, and health care team feedback.
  • Report patient needs/programs and observations regarding patient's condition to the physician
  • Work collaboratively with other members of the healthcare team in coordination of the patient's care as evidenced by other department feedback.
  • Participates in staff development programs.
  • Takes direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
  • Participates in continuing education activities and obtains required contact hours.
  • All other duties as assigned.

Requirements
Required Qualifications:
  • 1-3 years relevant clinical experience or current valid medical assistant credentials
  • Job experience that demonstrates an understanding of medical terminology
  • Prior job knowledge and use of electronic medical records.
  • Basic Life Support (BLS) required, must obtain within first 90 days of employment

Preferred Qualifications
  • Graduation from a medical assistant program
  • High school diploma or GED preferred
