Primary Care/ Family Physician South Bend, IN COMPENSATION: $240,000-$270,000 Plus 10% bonus We are looking for a full-time relationship-centered family Primary Care Physician to add to our team at our near-site employer-sponsored primary care clinic South Bend, IN. We provide high-quality, proactive primary care to adults and children in partnership with local employers or unions. We focus on prevention and wellness, acute and chronic disease management, and maximizing provider-patient relationships. We seek providers who are passionate about providing full-spectrum primary care resulting in top-notch clinical quality, outstanding service, and increased overall health outcomes. Role: Compensation: $240k-$270k plus 10% of base salary bonus potential Location: South Bend, IN Need: 1 primary care physician Full range of Family Practice Can take care of patients without copays and deductibles Patient volume: Avg. 10-12 Patient demographics: 50/50 men/women, (ages 2+) Clinical Support Staff: 1-NP, 1-PA, 4-MAs Clinic Hours: M-F Flexible schedule. Can do 4x10hr shifts Position typical day in the life Full primary care Acute Annual physicals Well womens visits Procedures - normal procedures - dispense onsite, labs Chronic disease management and coaching APP Collaborating Physician Benefits: Health, Dental, Vision, Wellness program (both spouse or domestic partner) PTO 5 weeks 401k match 100% of your contribution up to 3%; plus match the next 2% of your contribution at 50% up to 5% with a 3-year vesting schedule CME for PCP 5 days and $3,000 Maternity & Paternity leave: Ambassadors who give birth receive 6 weeks of full pay and benefits. Ambassador not a primary care giving role receive 2 weeks to support their partner and bond w/baby Adoption: same as maternity and paternity leave Medical licensing and DEA fully reimbursed Requirements: MD or DO Residency Trained IM/FM or IM-Peds (Or IM with some Peds exp, Urgent Care ok) Board Certified/ Board Eligible South Bend Indiana.
