The Department of Psychiatry and Behavioral Health at Greenwich Hospital seeks to recruit an advanced practice registered nurse to join our adolescent psychiatry team. Located at our Greenwich Office (500 West Putnam Avenue), the APRN will act as a prescriber for adolescents engaged in the departments intensive outpatient program (IOP) and for adolescents and young adults seeking pharmacotherapy. The APRN will work collaboratively with department and system physicians and the treatment team to provide diagnostic and therapeutic services for patients. What sets this role apart is the opportunity to work side-by-side with other staff on an interdisciplinary team, follow patients across IOP and outpatient settings, and provide clinical care on a team that is embedded within a larger department of psychiatry and behavioral health. This is an ideal role for an APRN who enjoys structured program work, values real-time collaboration, and wants the satisfaction of building ongoing therapeutic relationships in outpatient care.
The intensive outpatient program for adolescents
As a valued member of our team, you will: Provide assessment, triage, and therapeutic management of patient/family needs throughout the process as an integral part of an interdisciplinary team. This involves completion of initial medication evaluations for IOP admissions as clinically indicated and subsequent follow-up medication management visits. Work in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols, and clinical pathways, and may provide autonomous team-based care. Obtain medical histories, performs physical examinations when necessary, reviews/orders diagnostic/therapeutic tests and radiologic results, formulates and manages patient treatment plans. The APP will educate patients and families on care and available resources, in collaboration with the interdisciplinary team. This includes admitting instructions, treatment plans, potential side effects, and discharge plans. The APP, as part of an interdisciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner. Participating in team-based case conferences, treatment planning, and coordinating discharge planning IOP Days (Mondays, Tuesdays, Thursdays) are structured around group times to prioritize clinical needs for evaluation and follow up EDUCATION
