Job description

Hospice Registered Nurse (RN) | Compassionate Care Where It Matters Most

Are you a registered nurse who is passionate about providing comfort-focused care and supporting patients and families through every stage of the hospice journey? Join Ohio's Hospice and become part of a collaborative team dedicated to delivering exceptional end-of-life care with dignity, respect, and compassion.

As a Hospice Team Nurse Registered Nurse, you'll manage and coordinate patient care in homes, assisted living communities, skilled nursing facilities, and other care settings throughout our service area. You'll work closely with physicians, social workers, chaplains, hospice aides, and other interdisciplinary team members to develop individualized plans of care that prioritize comfort and quality of life.

This is a field-based position supporting patients served by our Lancaster and Newark locations.

Schedule
  • Full-time, Monday through Friday
  • Hours: 8:30 a.m. – 4:30 p.m.
  • Shared weekend on-call rotation (approximately one weekend every six weeks)
  • Holiday rotation required
  • Daily travel throughout the Lancaster and Newark service areas
What You'll Do
  • Complete comprehensive nursing assessments and develop individualized plans of care.
  • Manage pain and symptoms using evidence-based hospice practices.
  • Educate patients, families, and caregivers about disease progression, medications, and available hospice services.
  • Coordinate care with the interdisciplinary hospice team to ensure seamless patient support.
  • Maintain accurate and timely electronic clinical documentation.
  • Advocate for patients while promoting comfort, dignity, and quality of life.
  • Build meaningful relationships with patients and families during one of life's most important transitions.
Qualifications
  • Active Ohio Registered Nurse (RN) license in good standing with no disciplinary action.
  • Current CPR certification.
  • Hospice, home health, or palliative care experience preferred.
  • CHPN (Certified Hospice and Palliative Nurse) certification is a plus. Ohio's Hospice encourages professional development and provides certification opportunities for eligible team members.
  • Strong assessment, critical thinking, and clinical decision-making skills.
  • Experience documenting in electronic medical record (EMR) systems.
  • Valid Ohio driver's license, reliable transportation, and proof of current auto insurance.
  • Ability to successfully complete a criminal background check and 10-panel drug screening.
