Heights and Hills, an affiliate of Greenwich House, is a community-based agency providing services to Brooklyn's older adults and their families for over 50 years, has an opening for a qualified, committed MSW to work full-time in our Case Management program. Our mission, to promote successful aging in the community, is guided by the beliefs that older adults are assets to their communities; are entitled to live as independently as possible, with dignity and choice; and that professionally delivered home and community-based services for older people are essential. We serve homebound older adults and their families in 18 Brooklyn neighborhoods.


The Opportunity

The Case Management Supervisors report to the Case Management Program Director. As one of three Supervisors, each is responsible for overseeing the day-to-day operations of the Case Management Program, providing supervision and support to Case Managers, monitoring service quality and documentation, and ensuring program goals and agency standards are met.


The Supervisors keep the Program Director informed of staff needs, morale, operational issues, and program effectiveness. They also collaborate on program planning, quality improvement, staff development, and represent the agency in community partnerships and interagency meetings.

Responsibilities

  • With the other Case Management Supervisors, oversee management of Department for the Agingfunded-Case Management program
  • Supervise five case managers in their daily work
  • Work with Case Management Program Director and Intake Coordinator to assign cases
  • Review case records of case management staff for DFTA standards compliance and clinical appropriateness
  • Work with other supervisory staff and Case Management Program Director toidentifystaff needs and develop in-servicetrainingsto meet those needs
  • Supervise and coordinate activities of student interns
  • Representthe agency at community meetings
  • Reach out to community leaders, communityorganizationsand service providers to make them aware of Heights and Hills services, as well as toidentifythose in need of services andidentifygaps in service in the community
  • Forge community partnerships to enhance the life of seniors in the community
  • Work with the Case Management Director and other staff on special projects
  • Compile statistics, prepare reports, keepattendanceand other required records
  • Perform other duties asrequired

What You’ll Bring

  • LMSW or LCSW with two years of experience working with the elderly
  • Have an awareness of the biological, psychological, and social aspects of aging and the impact of disabilities and illnesses on aging and the family
  • Excellent organizational skills
  • Strong communicationskills
  • Knowledge of community resources and public benefits eligibility
  • Proven ability to work both independently and as part of a team
  • SIFI Certified or eligible (3 yrs post Masters)
  • Must be able to travel on public transportation to do community presentations and home visits with staff
  • Prefer bi-lingual and cultural competency in Spanish, Chinese or Haitian Creole.
  • Prefer Knowledge of Department for the Aging case management standards
  • Prefer Familiarity with theVivedatabase

Salary Range

The expected base salary for this position is $76,000 - $78,000 annually. This range reflects the anticipated compensation at the time of this posting and may be adjusted based on experience, education, and organizational needs.
