Job Summary

The Case Management Extender coordinates post-acute care authorization requests for assigned payors, ensuring timely submission and follow-up through determination. This role communicates authorization outcomes to case managers, physicians, and post-acute providers, and documents all activity in the electronic medical record. The Extender facilitates peer-to-peer reviews, supports denial and appeal processes in collaboration with the care team and families, and monitors Bridge 2 Rehab cases to ensure efficient resolution and minimize financial liability.

Minimum Requirements

High School or GED required

Experience with Word, Excel, EPIC, RightFax and ONBASE strongly preferred
