Job description

Position Title: Facility Case Management DirectorJob Summary:

As the Facility Case Management Director, you will have overarching responsibility for managing and coordinating department activities. Your role encompasses ensuring staff compliance with organizational policies and external regulatory agencies, while also leading efforts to integrate patient care and discharge planning processes across hospital departments and external agencies. Your leadership will drive continuity of care and optimize clinical resource utilization. With accountability for daily hospital case management operations, you will engage interdisciplinary teams and external stakeholders to achieve established outcomes.

Responsibilities:
  • Direct and evaluate departmental operations, including case management model, staffing, use of information technologies, onboarding, and staff competencies to achieve performance and quality objectives.
  • Allocate resources to effectively staff the department and meet productivity and quality goals.
  • Oversee core case management functions and practice, ensuring adherence to care coordination and discharge planning processes with a focus on timeliness, quality, and documentation.
  • Collaborate with the Facility CFO and Division CM Leader to achieve established goals and expectations.
  • Assess and improve departmental performance by evaluating operational processes, analyzing data, and implementing sustainable performance improvement activities.
  • Ensure compliance with policies and standard operating procedures (SOP).
  • Establish working relationships with key stakeholders, including CMO, CNO, ancillary service leaders, Ethics amp; Compliance, and Legal.
  • Practice and adhere to the "Code of Conduct" and "Mission and Value Statement."
  • Perform other duties as assigned.
Qualifications:
  • Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
  • Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
  • 5-7 years of experience in overall acute care Hospital Case Management.
  • 2+ years of experience in case management leadership.
  • Experience in a leadership CM role in a large acute care facility, preferably Trauma Level, is essential.
  • Polished demeanor with excellent communication skills and the ability to multitask effectively.
