(FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance use disorder, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

Family Guidance Centers, Inc seeks to support the behavioral health of individuals, families, and communities through our commitment to innovation, accountability, and compassionate care to facilitate positive, lasting change. Looking for candidates with personal lived experiences regarding substance use disorder and recovery. Essential Duties: Support patients accessing care on FGC's mobile medication unit in [Cook County/Harvey], including conducting initial assessments, arranging transportation, and ensuring patients are able to access sustained treatment at a community-based provider.

Deliver medications on the mobile unit to patients already enrolled in treatment services. Conduct outreach to people who use drugs, individuals experiencing homelessness, and community members. Oversee and support services delivered on FGC's Mobile Van, including the completion of the Government Performance Results Act [GPRA] data collection instrument.

Support positive customer service, which encourages quality patient outcomes. Document services provided in the individual patient record in a timely fashion and in accordance with IL.DASA, Federal and FGC standards. Describes FGC programs, services and eligibility requirements in laymen's terms to people struggling with opioid and alcohol addiction and those at risk of overdosing, as well as family members or friends of people struggling with addiction, informing and assisting them to gain immediate access to treatment

Communicates daily with FGC hospital-based medical detox staff, with the medication assisted recovery mobile van team, with FGC program managers at our MAR outpatient centers, and intake staff at all program locations, to coordinate access to treatment and broker transportation of community referrals to FGC service locations. Work Schedule: Monday - Friday (7:00a - 3:30p) Work Location: 15400 S. Page Ave., Harvey Pay Rate: $20.00 - $21.00 per hour Family Guidance Centers, Inc

Familiarity with federal and state confidentiality laws is essential. Certification as CRSS is preferred.
