We are currently seeking Direct Service Professional (DSP), who will work along side a nurse in a 1:1 setting; who is passionate, purpose-driven, and committed to excellence in home healthcare.

Key Responsibilities
Direct Care & Support:
  • Provide individualized support with activities of daily living (ADLs), including bathing, grooming, toileting, feeding, and mobility assistance.
  • Assist individuals in participating in community activities, promoting inclusion and socialization.
  • Foster independence and skill-building through everyday tasks and life coaching.
  • Offer respectful behavioral redirection when necessary, following support plans and agency protocols.

Health & Safety:
  • Monitor for changes in physical, mental, or emotional health and report observations promptly.
  • Support medication administration and document compliance (if certified and applicable).
  • Ensure a clean, safe, and supportive home or community-based environment.

Documentation & Communication:
  • Accurately document daily activities, incident reports, and progress notes in accordance with Electronic Visit Verification (EVV) standards.
  • Maintain confidentiality and compliance with HIPAA, Chapter 6100 regulations, and agency policies.
  • Collaborate with supervisors, families, and team members to coordinate care and advocate for individual needs.

Requirements
  • High School Diploma or GED (Required)
  • 1 -3 years of experience working in home care setting
  • Valid Driver's License, clean driving record, and reliable transportation
  • Ability to pass all background checks and drug screenings
  • First Aid/CPR Certification
  • EVV compliance knowledge or willingness to learn (training provided)
  • Strong communication, empathy, patience, and decision-making skills
