Job description

Eminence Home Care is currently seeking compassionate, dependable, and dedicated Direct Care Workers (DCWs) to provide quality care and support to individuals in a community-based setting. DCWs play a vital role in assisting participants with daily activities while promoting independence, dignity, and overall well-being.

Responsibilities
  • Provide direct care and support to participants in accordance with their individualized care plans.
  • Assist with activities of daily living (ADLs) as needed.
  • Ensure the safety, comfort, and well-being of participants at all times.
  • Encourage engagement in activities and social interaction.
  • Maintain accurate documentation and communication with supervisors.
  • Follow all agency policies, procedures, and participant rights guidelines.
  • Report concerns, incidents, or changes in participant condition promptly.
Schedule
  • Monday through Friday
  • Center Hours: 8:00 AM โ€“ 4:30 PM
  • Full-time and part-time opportunities may be available.
Qualifications

To be considered for employment, applicants must provide or complete the following:

Required Documentation
  • Signed Job Description
  • Valid Certification and/or License (if applicable)
  • OIG/SAM Clearance
  • 2-Year Pennsylvania Resident Attestation
  • Fingerprint Clearance Letter (required if not a 2-year Pennsylvania resident)
  • Proof of MMR Vaccination or Titer Showing Immunity
  • Completed Health Assessment Form (to be reviewed and approved by Kinship HR prior to first day)
Additional Requirements
  • Must pass all pre-employment screening requirements.
  • Must be authorized to work in the United States.
  • Must be reliable, professional, and able to work independently and as part of a team.
  • Previous direct care, caregiver, DSP, CNA, HHA, or healthcare experience is preferred but not always required.
