Part-time Care Manager

Salary based on experience

All Care Wellness is a privately owned mental health provider offering compassionate patient centered care with both in home and outpatient settings. Our goal of treatment is to promote physical, mental, emotional, and spiritual well-being by teaching the patient how to work with him or herself naturally with or without the use of psychiatric medication. We also manage a variety of mental health issues with an integrative approach or practicing psychiatry by blending traditional medicine with holistic therapies.

Some of the areas we can assist with include anxiety, substance abuse, panic disorders, obsessive compulsive disorders, post-traumatic stress disorder, ADHD, life skills, career, and relationship issues. Our practice also offers individual psychotherapy, medication management, home visits, and group therapy in addition to the Group Adult Foster Care Program services.

We are responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with relevant Agency policies and industry standards.

All Care Homecare, LLC care manager's primary function will be to provide support to members enrolled in the Group Adult Foster Care Program. This is a home care program so much of the time will be spent traveling to members' homes. It is therefore imperative that the Care Manager has reliable transportation.

Care Manager Responsibilities

  • Completion of biopsychosocial for each member
  • Bi-monthly home visits with each of the members
  • Maintain member records, charts, and other pertinent information
  • Participate in case conferencing
  • Provide direct member care as needed
  • Coordinate care with other treatment providers
  • Evaluation of direct participant care performed by the Direct Care Aide
  • Develop, along with the nurse, the member plan of care to be carried out by the Direct Care Aide (DCA)
  • Collect and document members current health providers and make additional referrals as necessary
  • Work as part of a multidisciplinary team alongside DCA, Nurse, and Program Coordinator along with other treatment providers the members may see
  • Respond to telephone inquiries relating to members requests and/or referrals for additional services.
  • Follow ups on doctor's appointments, medical releases, and medical equipment

Essential Skills

  • Skilled in assessing, planning, implementing, documenting, coordinating, and managing member care in keeping with facility standards
  • Knowledgeable in communicating and collaborating with members, family members, physicians and other health care professionals in achieving the best quality of care for the members
  • Thorough understanding of assessing members to recognize suitable community interventions
  • Ability to handle emergency situations in an opportune and safe manner
  • Demonstrate leadership skills to efficiently supervise skills of the Direct Care Aide
  • Proficient in entering, accessing, and retrieving patient data, and creating customized reports
  • Listening/attending skills: clarifying, paraphrasing, summarizing, reframing, non-verbal attending
  • Research, including practice evaluation
  • Policy practice
  • Effective use of supervision and consultation
