(NON-EXEMPT)

The position description expires one year from effective date unless renewed after one year. Renewals are effective in one-year increments.

Position Purpose: The Care Coordinator (CSU) will assist individuals admitted in the Crisis Stabilization Unit (CSU) with behavioral health needs through admission and after discharge. This includes services and support that affect a person’s overall well-being, such as primary physical health care, housing, and social connectedness.

Position Requirements / Qualifications:

Education/Experience:

Graduate from an accredited College or University with a minimum of a bachelor’s degree in the human service filed and have a minimum of one year of full time experience working with children and adolescents experiencing serious mental illness and in which the person provided case management, counseling, training, therapy, and/or guidance to children, adolescents and their families. Incumbents must be at least 21 years of age, have maintained a valid Florida driver’s license for at least one year, and must be able to pass a driver’s screening background check.

Licensure / Certification:

Maintain current CPR certification from the American Heart Association.

Skills / Ability:

Possess knowledge of community resources, Community-Based Organizations, and private providers of services to optimize the functions of individual eligible to receive management services. Good documentation skills, computer literacy, ability to work independently, excellent interpersonal and communication skills. Sensitivity to cultural, religious, racial, disability and gender issues.

Environmental conditions:
