Job description

Care Coordinator (CSU)

The Care Coordinator (CSU) will assist individuals admitted in the Crisis Stabilization Unit (CSU) with behavioral health needs through admission and after discharge. This includes services and support that affect a person's overall well-being, such as primary physical health care, housing, and social connectedness.

Education/Experience: Graduate from an accredited College or University with a minimum of a bachelor's degree in the human service field and have a minimum of one year of full time experience working with children and adolescents experiencing serious mental illness and in which the person provided case management, counseling, training, therapy, and/or guidance to children, adolescents and their families. Incumbents must be at least 21 years of age, have maintained a valid Florida driver's license for at least one year, and must be able to pass a driver's screening background check.

Licensure / Certification: Maintain current CPR certification from the American Heart Association.

Skills / Ability: Possess knowledge of community resources, Community-Based Organizations, and private providers of services to optimize the functions of individual eligible to receive management services. Good documentation skills, computer literacy, ability to work independently, excellent interpersonal and communication skills. Sensitivity to cultural, religious, racial, disability and gender issues.

Responsible To: Coordinator of Special Program

Position Number: Interrelationships: Maintains open communication lines with supervisors, peers, staff, and the public.

Environmental conditions: Position is required to work in a temperature-controlled environment; Program office approximately 25% of the time. Travel to client homes, schools and community locations require approximately 75% of the time. Potential exposure to violent situations. Potential exposure to airborne/blood borne pathogens or other potentially infectious materials.

Performance / proficiency standards: Performance standards are integrated within the position responsibilities. The principal functions of the position identified shall not be considered as a complete description of all the work requirements and expectations that may be inherent in the position.

Supervision provided to position: Direct supervision is provided by the Coordinator of Special Programs to the degree to which is evaluated on an individual basis.
