Job description

Job Title: Licensed Master Social Worker (LMSW)

Location: New York, NY 10016
Schedule: Monday - Friday | 9:00 AM - 5:00 PM (1-hour lunch)
Hours: 35 hours/week
Contract Type: Contract (Interview via WebEx/Phone)


Position Summary:

The Licensed Master Social Worker (LMSW) will provide social work services in an inpatient hospital setting, supporting patients in medicine, surgery, or rehabilitation units. This role focuses on care coordination, psychosocial assessments, discharge planning, and collaboration with interdisciplinary clinical teams to ensure safe and effective transitions of care.


Key Responsibilities:
  • Conduct psychosocial assessments for inpatient populations.

  • Provide emotional support, counseling, and crisis intervention as needed.

  • Participate in discharge planning and care coordination.

  • Collaborate with physicians, nurses, case managers, and interdisciplinary teams.

  • Connect patients and families with appropriate community and hospital resources.

  • Document assessments, interventions, and plans of care in the EMR.

  • Advocate for patient needs and ensure continuity of care.

  • Maintain compliance with hospital policies, NYS regulations, and ethical standards.

  • Participate in team meetings, case reviews, and quality improvement initiatives.


Required Skills & Experience:
  • Minimum of 1 year of experience as a Licensed Master Social Worker.

  • Inpatient medicine, surgery, or rehabilitation experience preferred.

  • Hospital-based social work experience strongly preferred.


Education:
  • Master's Degree in Social Work (MSW) - Required.


Licensure & Certifications:
  • Active Licensed Master Social Worker (LMSW) license issued by NYSED (Primary Source Verification required).


Schedule Notes:
  • 9:00 AM - 5:00 PM, Monday - Friday

  • 35 hours per week (1-hour lunch)

  • Interviews conducted via WebEx or phone
