Job description

Morrison Center is seeking a Contracted Case Manager State-wide with offices in Brewer, Oakland, Westbrook, and Kennebunk, Maine. This is a REMOTE position and also to have hands on in person engagement. MUST BE IN MAINE!
This is a temporary contract position for a defined term and does not constitute employment.

Requirements of the Coordinator Program Assistant:
Candidates should possess relevant case management experience and meet all applicable professional requirements and qualifications.
  • Bachelor's degree in any field (required)
  • Minimum 1 year of experience working with adults in human services
  • Experience with PCPs, family systems, and the Evergreen system is preferred
  • Strong communication, time management, and problem-solving skills
  • Ability to work independently with a high level of professionalism
  • Microsoft Office Suite

Contract Details:
  • Contract Type: Independent Contractor
  • Pay: $65 per billable hour!
  • Contract Duration: a 4-month independent contract engagement, July - October
  • Schedule: Flexible hours (Monday-Friday)
  • Location: Remote with travel for home/community visits in Maine

Responsibilities of the Community Case Manager:
  • Support adults with intellectual and developmental disabilities in achieving their personal goals
  • Develop and manage Person-Centered Plans (PCPs), assessments, and service goals
  • Ensure all documentation is accurate and compliant with Department of Health and Human Services requirements
  • Conduct monthly home visits and maintain open communication with clients and families
  • Perform research and collaborate with service providers to ensure effective supports are in place
  • Serve as a trusted resource and advocate in the community
