Job description

Outreach Case Manager

Summary

The Case Manager will be responsible for Homeless Outreach and Case Management Services, conducting various job duties. The Case Manager will provide client services and resource referrals as needed.

Position Type: Non-Exempt/Full-Time

Reports To: Program Director of Housing and Homeless Services

Essential Functions

· Conduct street outreach as scheduled by the Program Director

· Conducting Street Outreach to fulfill a contract with HUD

· Responsible for linking participants to referrals and community services in EBR

· Complete intake and initial assessment.

· Data entry into HMIS (Homeless Management Information System)

· Inputting Coordinated Entry Data

· Driving the mobile outreach van

· Assist program individuals in obtaining community support, resources, and services

· Complete monthly and quarterly program reports

· Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time cards, requesting edit sheets, etc)

· Report any significant concerns to the supervisor

· Maintain training requirements

Required Education and Experience

Bachelor’s degree in the Human Service Field

One-year experience working with homeless population and/or individuals with disabilities is required

Time management and multi-tasking skills are a must

Have valid driver’s license and current vehicle insurance

While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
