What You’ll Do as part of a team:

  • Assess patient needs and develop personalized care plans after visiting them in the hospital, home or rehabilitation settings.
  • Coordinate with healthcare providers, families, and community resources to ensure seamless care.
  • Monitor patient progress and adjust plans as necessary to meet evolving needs.
  • Advocate for patients, ensuring they receive the best possible care.
  • Provide education and support to patients and their families regarding treatment plans and health conditions.

What We’re Looking For:

  • Current RN License (in the state you’ll be working).
  • Current, unrestricted driver's license and auto insurance
  • Ability to travel within 100+ miles of your home for patient appointments, hospital and rehabilitation visits
  • Minimum of 2 years of trauma or ICU nursing experience, with some experience in case management.
  • Ability to work independently and manage your schedule effectively.
  • Comfort with computer skills and healthcare software data entry
  • A highly organized individual with a focus on timeliness and attention to detail
