The Hospice Community Outreach Specialist educates target groups external to the Franciscan Hospice for the purpose of maintaining continuity of care for clients in transition to the hospice environment. This role develops, coordinates, and implements successful community outreach marketing strategies and events based on an understanding of the market, the hospital, its clients and its competitors.

WHAT YOU CAN EXPECT
  • Monday to Friday- 8am to 5pm
  • Coordinates CBISA reporting and troubadour activities to collect information and report on community involvement and program impact on improving health outcomes in local communities.
  • Coordinates the Speakers Bureau to identify and provide speakers on various health and wellness topics to internal and external events.
  • Plans and coordinates booth setup and staffing for health screenings.
  • Develops and implements specific marketing programs and strategies for maintaining contact with homecare and hospice referral sources.
  • Tracks and evaluates effectiveness of marketing strategies.
  • Maintains effective follow-up on prospective referral sources through personal visits, phone calls, and written communication.
  • Develops long-term relationships with key decision makers in the organizations within target market.
  • Maintains contact management database to provide historical record of contact demographic data for each referral account.
  • Provides education and awareness of home care and hospice services to medical, nursing, and other professional colleagues.
  • Responds and follows up on all complaints and concerns from referral sources and physicians and establishes best practices for the future.

QUALIFICATIONS
  • Bachelor's Degree- Health Promotion, Marketing, Sales or Related- Required
  • 2 Years- Health Care Related Sales and Marketing Experience- Required
