East Liberty Family Health Care Center (ELFHCC) is a mission-driven Federally Qualified Health Center committed to providing whole-person, high-quality health care to the communities we serve. We are seeking a compassionate, community-focused Community Health Worker (CHW) to support patients in accessing care, addressing social determinants of health, and improving overall wellbeing.
Position Summary
The Community Health Worker plays a vital role in connecting patients to resources, providing education, and building trusting relationships that support healthier outcomes. This role includes both clinic-based and community-based work, including home visits, outreach events, and coordination with partner organizations.
Key Responsibilities
  • Build supportive, trusting relationships with patients and families
  • Conduct outreach, home visits, and follow-up to ensure patient engagement in care
  • Identify and address social determinants of health (housing, food access, transportation, utilities, insurance, etc.)
  • Provide education and coaching around chronic disease management, preventive care, and healthy living
  • Assist patients in navigating the health care system and connecting to community resources
  • Serve as a liaison between patients, providers, and community organizations
  • Document encounters and maintain accurate records in the electronic health record
  • Participate in community events, screenings, and health promotion activities
  • Support ELFHCC's mission by promoting equitable, community-centered care
Qualifications
Required:
  • High school diploma or equivalent
  • Certified Community Health Worker (CHW)
  • Strong communication, interpersonal, and relationship-building skills
  • Knowledge of local communities and willingness to conduct community-based outreach
  • Ability to work collaboratively in a diverse, mission-driven environment
  • Valid driver's license and reliable transportation

Preferred:
  • Prior experience in community health, case management, outreach, or social services
  • Familiarity with FQHC settings, health equity initiatives, or trauma-informed care
  • Bilingual skills are a plus
