JOB DESCRIPTION
POSITION: Community Health Worker
LOCATION: Red Cliff Community Health Center
WAGE: $20.00 - $22.00 per hour
SUPERVISOR: Community Health Supervisor
THIS IS A REGULAR FULL-TIME NON-EXEMPT POSITION
JOB SUMMARY: The Community Health Worker helps individuals and families to improve their health, increase their access to health and social services, and reduce health inequities.
Examples of Duties
DUTIES AND RESPONSIBILITIES:
  1. Participate in community health outreach activities:
    1. Advise clients or groups on issues related to self-care, such as diabetes management.
    2. Identify high risk members of target population with overdue health needs.
    3. Distribute culturally appropriate educational materials such as flyers, brochures, or other material to inform members of the target population.
    4. Attend community events to engage and strengthen relationship between community and health service system.
  2. Transport or accompany clients to scheduled health appointments or referral sites.
  3. Optical duties to include ordering, fittings, repairs, dispensing, and optical testing (as needed).
  4. Provide feedback to health service providers regarding improving service accessibility.
  5. Provide case management type services including periodic and on-going assessments, referrals to other resources and assistance with overcoming barriers to access.
  6. Participate as a member of the Community Health, respiratory season response team which may include testing, education, and support.
  7. Collaborate with Wrap Around Services to assist clients with applying for social services, including Medicaid, Long Term Care, Foodshare etc.
  8. Documentation of patient visits, transports in the Electronic Health Record as required.
  9. Maintain strict confidentiality and safeguard privacy of patients in common areas.
  10. Advance job knowledge and skills through continuing education efforts with the approval of supervisor and/or Health Center Administration.
  11. Attend staff and other meetings, in-services, and other events as directed by supervisor.
  12. Maintain a clean and safe physical environment. Alert administration and/or supervisor to problems and difficulties, as circumstances may warrant.

The duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the supervisor.

Typical Qualifications
KNOWLEDGE:
  1. Organization skills and attention to detail.
  2. Computer skills; data entry, Excel program, Word, Publisher.

QUALIFICATIONS:
  1. Minimum of high school diploma, HSED or GED required.
  2. Completion of Community Health Worker Program via Area Health Education Careers (AHEC) required within six months of hire.
  3. One year experience working in a clinical setting - preferred.
  4. Current CPR certification or willing to accomplish within 90 days of employment.
  5. This position must possess and maintain a current valid Driver License. Must be eligible to be placed on the tribe's vehicle insurance.

Supplemental Information
PERSONAL CONTACTS: Considerable daily contact will be made with the general public, tribal leadership, Program Directors and staff, consultants, and the Tribal Community.
PHYSICAL REQUIREMENTS: The duties assigned to this position involve bending, stooping, lifting, and carrying. Items may be placed in overhead storage or shelving.
WORK ENVIRONMENT: Red Cliff Community Health Center; office and clinic settings. Exposure to hazards of the health care industry. All Tribal Buildings are smoke free.
TRAVEL REQUIREMENTS: The ability to travel and attend meetings and trainings; overnight and out of town, both locally & nationally.
