Love and Loyalty Home Care is seeking an energetic, professional, highly motivated Community Relations & Business Development Coordinator to help grow our home-care client base and strengthen professional referral relationships throughout the communities we serve.
This is primarily a field-based business-development position, not a desk-based social-media marketing position.
The successful candidate will spend a significant portion of their working time in the community developing relationships with healthcare professionals, social workers, case managers, discharge planners, attorneys, senior-service professionals, community organizations, and other qualified referral partners.
Our objective is not simply to increase brand awareness.
Our objective is to develop credible professional relationships that generate qualified referrals and new home-care clients.
Responsibilities
The Community Relations & Business Development Coordinator will:
• Conduct regular face-to-face visits with qualified referral sources.
• Develop relationships with hospital case managers, social workers, discharge planners and care-transition professionals.
• Visit rehabilitation and skilled nursing facilities.
• Develop relationships with physician practices and healthcare organizations.
• Develop relationships with hospice and appropriate home-health professionals.
• Develop relationships with elder-law and estate-planning attorneys.
• Develop relationships with geriatric care managers/Aging Life Care professionals.
• Develop relationships with senior communities and senior-resource organizations.
• Develop relationships with Departments of Social Services, Community Services Boards, disability organizations, behavioral-health organizations, and appropriate children's/family-service organizations.
• Professionally introduce Love and Loyalty Home Care and clearly communicate our services.
• Identify opportunities for Medicaid and private-pay clients.
• Specifically develop opportunities for dementia care, overnight care, high-hour cases, 12-hour care and 24-hour private-pay care.
• Conduct meetings and presentations with professional organizations.
• Attend appropriate networking and community events.
• Identify unmet home-care needs in the community.
• Ask referral sources what difficulties they experience securing care for patients and families.
• Conduct consistent professional follow-up.
• Coordinate daily with our virtual business-development support.
• Document all meaningful visits, conversations, opportunities and follow-up activities in our existing business-development system.
• Immediately escalate active client opportunities to management.
• Help move opportunities through:
