Job description

JOB SUMMARY

The program’s Social Worker/Case Planner is responsible for the comprehensive service coordination from initial assessment to obtaining appropriate and adequate services for each resident.  This includes planning the resident’s services, linking provider to persons in need of service, monitoring the services received in light of a resident’s service plan, and coordinating all phases of services, such as vocational, educational, residential and day programming. The social worker also arranges for appropriate transition services for a youth nearing discharge from the program. 

GENERAL RESPONSIBILITIES

  1. Write treatment plans, court reports, progress notes and individual crisis management plans.
  2. Complete all FASPs and Permanency Hearing reports according to agency guidelines.
  3. Facilitate home visits, family visits, sibling visits for the youth as well as communication between the youth in the program and family members.
  4. Assist family members in understanding the treatment for their child and how to apply these skills and interventions to other settings.
  5. Provide comprehensive service coordination from initial assessment to obtaining appropriate and adequate services for each resident.
  6. Arrange appropriate transition services for a youth nearing discharge from the program. Each residence will have its own Social Worker/Case Planner
  7. Perform all necessary activities assigned to further the agency’s mission.
  8. Ensure compliance with all job duties, assignments, and communication with regards to the Case Planner Job Description.
  9. Ensure compliance with Direct Supervisor as designated by immediate supervisor/designee.

PHYSICAL REQUIREMENTS

The employee frequently is required to walk.  The employee is occasionally required to keep balance and stoop, kneel, crouch, or crawl.
  • Computer – The Social Worker/Case Planner must be able to use a computer and able to learn software programs
  • Office Equipment- The Social Worker/Case Planner is required to operate a fax, copier, scan, and other office equipment.
  • Communication - The Social Worker/Case Planner needs to be able to speak, read and write fluently in the English language
  • Writing - The Social Worker/Case Planner must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Lifting - The Social Worker/Case Planner may frequently lift 0-25 lbs., occasionally lift 25-50 lbs., and rarely lift over 50 lbs.
  • Push/Pull - The Social Worker/Case Planner may frequently push/pull 0-25 lbs. of force, occasionally push/pull 26-45 lbs. force, and rarely push/pull over 45 lbs.
  • Sitting - The Social Worker/Case Planner may sit 50-60% of the working day.
  • Standing - The Social Worker/Case Planner may stand 10-20% of the working day.
  • Walking - The Social Worker/Case Planner may walk 10-20% of the working day.
  • Stooping -The Social Worker/Case Planner may occasionally stoop.  This is defined as bending body downward by bending spine at the waist.
  • Kneeling -The Social Worker/Case Planner may occasionally kneel.  This is defined as bending legs at knees to come to rest on knee(s).
  • Crouching - The Social Worker/Case Planner may occasionally crouch.  This is defined as bending body downward and forward by bending spine and legs.
  • Work environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee frequently works in an office environment. The noise level in the work environment is usually moderate due to talk and movement of the program participants and other co- workers.  The noise level can increase on occasion due to yelling or screaming by the participants of the programs.

QUALIFICATIONS

  • You have at least a Bachelor’s degree with experience in Child Welfare. Master’s degree is preferred.
  • You have a minimum of 2 years of experience in child welfare roles
  • You have professional maturity and demonstrate the ability to handle sensitive and confidential situations
  • You have outstanding interpersonal and communication skills
  • You are technology savvy with excellent command of a variety of computer software applications in word processing, spreadsheets, database, and presentation software (MSWord, Excel, Access, PowerPoint)

HOURS AND TIME OF WORK

  • The Social Worker/Case Planner is contracted for 35 hours a week Monday through Friday from 9:00 AM to 5:00 PM. Some evenings, weekends and early mornings may be required
