Job description

The Center for Youth: Started BY YOUTH FOR YOUTH. We partner with youth to realize their full potential, by creating opportunities, removing barriers and promoting social justice.
Overview:
During a crisis, such as an accident, sudden illness, or housing emergency, a caregiver can bring their children to the Crisis Nursery or Owen House. Staff provides free, temporary, professional care to birth to 14 years old.
The Crisis Nursery Family Resource Navigator will provide supportive case management service to families utilizing the program. They will conduct a needs assessment for families interested in receiving case management services. The Family Resource Navigator will assist clients with referrals to community resources.
Schedule: Full time. Day and evening hours
Responsibilities:
  • Crisis intervention and response
  • Outreach, screening, intake and assessment
  • Coordination, development, implementation, ongoing assessment of family's case management needs, service and plans
  • Referral readiness, information and facilitation
  • Transporting program participants, as required
  • Case management

Required Qualifications:
  • Associate's Degree in Human Services or related field AND 1 year related experience OR equivalent / Bachelor's Degree preferred
  • A valid drivers license
