We are offering a great opportunity for a full-time Case Management Supervisor.

Responsibilities

This position primarily serves as a Case Management Supervisor and is responsible for the day-to-day oversight of Mental Health Case Management (MHCM) services within assigned clinics or service areas. The Case Management Supervisor provides direct supervision and oversight of MHCM staff and service delivery and is responsible for ensuring that case management services are delivered in accordance with regulations and standards guiding service delivery, documentation, engagement, and caseload management.  This role serves as the first-line supervisor for case management staff and is accountable for monitoring staff performance, ensuring compliance with required activities (including monthly engagement, face-to-face contact requirements, and ISP development and review), and maintaining the overall quality and integrity of MHCM services. 

The Case Management Supervisor supports the Adult Services Manager by overseeing day-to-day operations of the case management program, addressing routine staff and service delivery concerns, and ensuring consistent implementation of program expectations across teams. This position is responsible for identifying and escalating clinical, operational, and staffing concerns to the Adult Services Manager as appropriate. Within the scope of the case management role, this position may support crisis de-escalation and coordination of care but does not provide clinical crisis intervention services.   This position must establish a welcoming environment for individuals with serious mental illness, substance use disorders, and co-occurring needs while promoting recovery and community integration, and maintain a welcoming, supportive, and productive work environment for supervised staff.

Required Education and Experience

  • Bachelor’s degree in human services or related field required
  • Qualified Mental Health Professional as defined by Department of Behavioral Health and Developmental Services required
  • Minimum of three (3) years of documented and continuous experience as a Qualified Mental Health Professional.
  • Minimum of three (3) years of experience in mental health case management or related services required
  • Qualified as a QMHP supervisor or the ability to become qualified within 30 days of being hired.
  • Demonstrated ability to support staff development, provide guidance, and assist with supervision or training activities preferred; demonstrated performance in documentation quality, productivity, and adherence to program expectations, as evidenced by prior performance measures and evaluations
  • Valid Driver’s License with a point balance of -6 or higher required

Preferred Experience

  • At least one year of supervisory experience is required.

Required Knowledge, Skills and Abilities

  • Knowledge of MHCM service delivery, including assessment, service planning, referral, coordination, and monitoring activities; knowledge of serious mental illness, substance use disorders, and co-occurring disorders; knowledge of community resources and service systems that support access to care; understanding of documentation, service authorization, and reimbursement processes; knowledge of staff supervision, consultation, and training techniques; understanding of crisis response systems and appropriate use of crisis de-escalation within the scope of the case management role
  • Excellent oral, written, and interpersonal skills
  • Demonstrated ability to support the day-to-day administration of a service delivery program, including monitoring staff performance, reinforcing workflows, and ensuring compliance with established requirements
  • Demonstrated ability to effectively utilize community resources to support service coordination and linkage; coordinate staff activities and service delivery; support individuals with co-occurring needs; and provide guidance to staff within the scope of MHCM services
  • Ability to exercise sound judgment and work with limited supervision
  • Demonstrated ability to sustain cooperative and effective working relationships with Greater Reach staff and community partners
  • Ability to maintain a high degree of confidentiality
  • Must be able to travel to other clinic sites and attend meetings as required
  • Must be available to address urgent issues and provide support within the scope of the role, including after business hours if needed

Special work conditions of this position

Ability to lift up to 35 pounds (ex. supplies for meetings, items delivered to exec admin, etc.).  Work evenings when required to address board/agency functions.  Travel throughout the Commonwealth including overnight when applicable.

Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.  Our Agency maintains a drug-free workplace.

Grade S-C9
