Inova Fairfax Hospital is looking for a dedicated Experienced Manager Case Management to join the Case Management Team. This role will be Full-Time, Day shift: Monday - Friday 8:30 am - 5:00pm. This position has hybrid capabilities.

The Manager of Case Management participates in clinical performance improvement activities. Collaborates and communicates with interdisciplinary teams in all phases of discharge and care planning processes. Communicates with patients, family, medical staff, care team members, students, supervisors and outside contact. Evaluates the process of initiating plans for discharge and/or transitions of care and ongoing case management needs for patients and/or families is required. Supervises staff on the coordination and facilitation of patient care progression throughout the continuum of care.

Manager Case Management Responsibilities:

  • Participates in the interviewing/hiring process by reviewing job profiles, reviewing candidate applications/resumes, developing behavioral interview questions, conducting and evaluating interviews in a timely fashion and providing feedback/recommendations on hiring decisions.
  • Ensures referrals are initiated to appropriate next level of care (e.g. acute care, home health care, hospice, SNF, LTAC, clinics, TCM), in a timely manner, and coordinates ordering of medical equipment/supplies for safe discharge planning or ongoing care planning.
  • Works with staff to facilitate family meetings with all patients as requested by patient, family or healthcare teams. Leads collaboration with physician and members of the interdisciplinary teams to facilitate care for designated caseloads.
  • Supports the contact of families, physicians or other healthcare team members in anticipation of specials needs of the patients or families, in a timely manner, to facilitate effective treatment plans. 
  • Provides educational materials and methods in support of team members' abilities to document appropriately to patients' level of understanding and motivation.
  • Ensures that patients/families are appropriately assessed for needs with a developed and individualized continuing care plan in collaboration with physician.
  • Educates staff on variances in patient responses and how to proactively adjust the care plan in coordination with the interdisciplinary team. 
  • Supervises staff on the screening process with patients within 24 hours of admission for appropriate services and interventions
  • Evaluates the team members under their supervision assuring proper knowledge gaps are closed and provided necessary education to develop the case manager to the highest level possible.
  • Participates in evaluation of metrics and problem solves with the case managers under their supervision to achieve operational goals and metrics
  • Performs additional duties as assigned.

Minimum Requirements:

  • Education: Requires a Bachelor's Degree in Nursing or Master's Degree in Social Work.
  • Experience: Three (3) years of experience in case management or relevant nursing experience to include utilization review, discharge planning, outcomes management, transitional planning, assessment, care planning and service implementation and two (2) years of direct supervisor/manager experience.
  • License: Must be licensed in the Commonwealth of Virginia to practice as a Registered Nurse (RN) upon start. 
  • Certification: Basic Life Support (BLS) for Healthcare Provider certification from the American Heart Association required upon start. 
    • Must have one of the following: Accredited Case Manager (ACM) or Certified Case Manager (CCM), or MCG/UR or, Certified Clinical Transplant Social Worker (CCTSW) pr N/ASW-CM (ASWCM) or licensed as a Social Worker (LCSW) in Virginia or eligible to practice on the Commonwealth of Virginia as a Social Worker within 12 months of hire.

Preferred Qualifications:

  • Three (5) + years of previous Inpatient (hospital) case management experience, case management discharge planning, and two (2) years supervisory/lead experience is highly preferred. Previous experience working through medically complex cases. Current holder of ACM or CCM, or LCSW is also highly preferred.
