Shelter House was established in 1981 as a grassroots responder to the homelessness crisis in Fairfax County.ย  Since the organization's inception, we have adapted and grown to meet community needs, all while working in partnership with government and private partners and community members. Today, Shelter House is a successful and reputable organization of over 200 employees committed to preventing and ending homelessness and domestic violence.ย  In the last year, our compassionate team of individuals changed the lives of nearly 2,000 individuals, half of which were children.ย  Our Culture is built from our Core Values of Inclusivity, Collaboration, Accountability, Respect, and Empowerment (iC.A.R.E.)ย 

About the Role:ย 

Senior Case Manager is responsible for leading the shelter case management team to provide comprehensive services to households experiencing homelessness to end their homelessness. The Senior Case Manager will carry a caseload at the hotel site. The position requires a client-centered approach that respects self-determination, involves them in decision-making, and empowers them to achieve their goals. Effective collaboration with community partners and service providers is essential for providing holistic support and addressing any gaps in care. The role also involves maintaining accurate documentation and preparing reports to ensure compliance with organizational policies and funding requirements. The role involves developing and implementing individualized plans to secure permanent housing.ย ย 

The Senior Case Manager will lead the team in prioritizing housing solutions, collaborate closely with the assistant director of programs, and ensure that both on-site and hotel locations maintain the necessary capacity to meet community needs. Additionally, the Senior Case Manager will provide direct support to Case Managers to enhance service delivery.ย 

Responsibilities:ย ย 

Participant-Centered Support and Supervision:ย 

  • Carry a caseload of participants at the off-site hotel locationย 
  • Develop individualized housing-first service plans and guide participants through ending their homelessness:ย 
  • Include housing location service, referral, and advocacyย 
  • Facilitate housing-relation tasks such as collecting necessary documentsย 
  • Assist with developing a rental assistance package for participants utilizing a variety of funding sources, properly document the distribution of funds, and lead the recertification process if additional funds to maintain housingย 
  • Develop and implement individualized plans to help participants secure and maintain permanent housing.ย 
  • Assist participants in securing basic needs items and services, including clothing, employment, childcare, etc.ย 
  • Empower and support participants in the successful completion of their identified goals.ย 
  • Provide services that are convenient and accessible for participants, including off-site visits.ย 
  • ย Lead the case management team in providing effective services to the participantsย 
  • Provide direct support to Case Managers, including phone screenings, diversion support, and guidance on complex cases.ย 
  • De-escalate any safety concerns promptly and compassionately.ย 
  • Complete and submit all documentation required for incidents before the end of the shiftย 

Collaboration & Administrative Supportย 

  • Function as a contributing member of a multidisciplinary team to ensure timely service deliveryย 
  • Assure that all relevant information about participants is disseminated to all appropriate staff members promptly.ย 
  • Monitors participants progress by regular, ongoing contact with individuals receiving services and contact with collaborative service providers both external and internal necessary assessments. Make referrals to support participants' goals and needs.ย 
  • Manage tasks and scanning as needed to have a complete and current chartย 
  • Participate in weekly staff meetings and training as directed by the supervisor.ย 
  • Collaborate closely with the Assistant Director of Programs, hotel staff, LHSC site, and intake and referral (I&R) team to ensure that bed capacity meets community needs.ย ย 
  • Proactively communicate caseload coverage and provide a brief of outstanding tasks when absent from work.ย 
  • Ensure accurate and timely entry of participants data into the Homeless Management Information System (HMIS) and oversee data entry by direct reports.ย 
  • Maintain HMIS accuracy and data clean-ups monthly.ย 
  • Facilitate ongoing reviews of participant's service plans, and leading meetings and supervision sessions as needed.ย 
  • Participate in intakes, on-call manager rotations, and support file audits and intake process reviews.ย

Requirements

Required:ย 

  • A bachelor's degree in human services/ related field or commensurate experienceย 
  • 3+ years of street outreach/case management/ direct services/other related experienceย 
  • Strong written and oral communication skillsย 
  • Ability to prioritize competing priorities and make sound judgementsย 
  • Ability to complete tasks while navigating frequent interruptionsย 
  • Ability to deliver creative, resourceful solutions to unique challengesย 
  • Strong organizational and time management skillsย 
  • Valid Driver's License and reliable transportationย 
  • Ability to Drive a 15-passenger vanย 
  • Willingness to work evenings and weekends and as neededย 

Physical Requirements:ย 

  • Annual TB Test is requiredย 
  • Ability to sit or stand for long periodsย 
  • Ability to lift items weighing 10-20 poundsย
