Job description

Case Manager

Case Managers are a critical part of the agency and must have experience working in the home health setting. Case Managers facilitate direct patient care and skilled nursing visits. Case Managers perform tasks such as medication management, wound care, vital sign monitoring, and patient education in accordance with the established plan of care.

We are looking for individuals that are dedicated and dependable. Case Managers must have the following:

  • Must hold a current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing.
  • Must have experience working in a home care agency.
  • Must have direct patient care experience.

Job Duties:

  • Complete patient care duties.
  • Follow patient plan of care and reevaluate as necessary.
  • Work scheduled shift to perform patient duties.
  • Complete clinical nurse notes in their entirety.
  • Complete skilled care services as applicable.
  • Additional duties as assigned.
